PRIMARY ELEMENT PERFORMANCE REQUEST QUESTIONNAIRE
1. Name and email address
2. Location (address, city, state if outside of Washington)
3. For fire performance requests only; public land, or privately owned property?
a. Has the owner of the property been informed of the use of fire and have they given consent?
b. Indoor or Outdoor?
c. If indoor, what is the approximate ceiling height (must be a minimum of 12 ft.)
4. Can we do a site visit prior to the show date, or do you have photos that you can send us of the performance area?
5. Do you have music you'd like us to perform to? Do you have a music player available or shall we supply our own music?
6. Is the performance area flat or sloped?
a. Lawn, cement, wood, carpeted?
7. Is the performance area relatively clear (within 15ft) of decorations, low-hanging branches, trees, bushes, rocks, structures (buildings, fences, decks...etc)?
8. Is there a particular routine or type of show that you would like to see?
9. Day or evening performance?
TO BE NOTED:
* Fire performance during the daytime is limited to fire-breathing only.
* Extra fees may apply if:
a. You need us to supply a sound system for our performance
b. The Fire performance is on public property, on which a permit is needed
c. Insurance is required
d. The venue is more than 30 miles travel time from the Seattle metro area
* Fire performance on public property within Seattle City limits requires permits from the Fire Department and sometimes permits from the Department of Parks and Recreation. On occasion, venues and parks also require insurance. Please contact us at the very least 2 weeks prior to the show date if we need to make arrangements with your City Fire Department, if you haven't already done so. The hiring party is responsible for making arrangements with your City Parks and Recreation department, if need be.
* It is suggested that you inform your neighbors that we use fire in our performance, particularly so that they don't call the local Fire Department mistakenly.
"BAD WEATHER" PLAN:
In the case of wet conditions, rain and/or heavy winds at showtime, you will have the option to:
a. be refunded in full, with the exception of fuel and sound system rental expenses
b. reschedule for another date
c. re-work the routine and cost on the spot to accommodate bad weather
